Peninsula Center Library Passport Services

We are authorized by the U.S. Department of State to process the DS-11 application.
  • This application is for Adults who do not wish to RENEW by mail, or may not be eligible to renew by mail.
  • For First Time Applicants and for Children or Teens whose passports are expiring or has expired.
We DO NOT PROCESS the DS-82 Renewal Application; the applicant mails the application directly to the passport agency if they can answer yes to the questions on the front of the application.

Important: The passport agency NOW requires the "Prior Version" of the DS-11 application until an updated version is available. Please submit DS-11 12-2020, with an Expiration Date of 12-31-2023

Page 1 of the DS-11 application must not contain errors. Corrections on the application made with whiteout, or by crossing out incorrect information, and writing the correction above or beside the error is not acceptable on page 1. Mistakes on page 2 can be "Neatly Crossed Out & Corrected".

  • Saturday 8:00 am - 3:30 pm
  • Sunday 8:00 am - 1:00 pm
